Slow them down to help not mess up the combo, set up the teleport to where you want to pull them, quicky teleport on top of your desired target and use Soul Chain, then let the magic happen.

Use right after setting up a delayed teleport with Mage Sandals and either Glacial Prisoning an enemy or using Frost Nova to get on top of an enemy.
Freeze the target, walk up to them and set up the delayed teleport up, Soul Chain them, let the magic happen. Simple!
